Only 1 registration should be completed per company.

The person who completes this registration should be the main contact person or administrator for the company.

If you are a sole trader/ owner operator you will need to complete both the registration and induction forms.

Use full personal and company names to avoid confusion and double ups (no nick names).

Contractor Safety Agreement

We confirm that we understand, and will comply with the requirements of the Work Health and Safety Act, all other applicable legislation and all conditions specified by the Shire of Dardanup

We undertake to utilise personal protective clothing and equipment when required. Minimum Shire of Dardanup requirement for PPE when not in an office environment is long pants, steel cap boots and hi-vis long shirts.

We undertake to allow our employees to attend all safety inductions at our expense, prior to the commencement of the contract.

We undertake to report all injuries, work related sickness, injury or damage to third parties and their property, hazards and near-misses to Shire of Dardanup promptly and will complete all necessary forms provided for this purpose by Shire of Dardanup’s WHS team.

We understand that any disputes relating to safety and health issues which cannot be resolved with Shire of Dardanup’s WHS team will be referred to Worksafe WA.

We undertake to leave all sites in a safe condition and maintain site security both when on and off site.
